Its readings are not a medical diagnosis, a laboratory test, or proof of the root cause of inflammation. Coaching and QEST4 do not replace evaluation or treatment by a licensed healthcare professional.
How your remote package works
1. Purchase this package and download the hair-sample instructions and contact form. A download link is also sent to the email address used at checkout.
2. Collect a small tuft of hair approximately 1 inch long. Place it in a zip-top bag. Write your full name, phone number, and email address on both the bag and the enclosed form.
3. Mail the labeled sample and completed form to Elite Nutrition Research, 6657 Frank Ave NW, Suite 300, North Canton, OH 44720.
4. Once we receive your sample, we will contact you to schedule your two-hour phone consultation.
